Palpation Inaccuracy
Evidence shows that manual palpation is unreliable and slow, particularly in emergency settings: providers take too long to detect a pulse correctly, and misidentify pulselessness in many cases.
Point-of-care ultrasound (POCUS) has been demonstrated to be much more accurate and rapid, routinely identifying perfusion sooner and more reliably than palpation. However, this requires a skilled and practiced provider who is dedicated to this task, while often relying on others to guide the resuscitation.
This gap matters because mistaking a weak but present pulse (a condition sometimes called either PseudoPEA or PREM) for true pulselessness can lead to different—and potentially harmful—treatment paths.
